02. தந்தையின் மரணச் சான்றிதழ் பிரதி
03. தாயின் பிறப்புச் சான்றிதழ் பிரதி
04. பெற்றோரின் திருமணப் பதிவின் பிரதி
05. தாயின் தேசிய அடையாள அட்டை பிரதி.
இத்துடன் இணைக்கப்பட்டுள்ள வின்னப்பப்படிவத்தை ஆங்கிலத்தில் பூர்தி செய்து மேற்குறித்த தெளிவான ஆவணங்களுடன் கீழ் கானும் முகவரிக்கு பதிவுத் தபால் அல்லது மின்னஞ்சல் ஊடாக அவசரமாக அனுப்பி வைக்கவும்.

1. Copy of the Child’s Birth Certificate.
2. Copy of the Father’s Death Certificate.
3. Copy of the Mother’s Birth Certificate.
4. Copy of the Marriage Record/ Certificate of the parents.
5. Copy of the Mother’s National Identity Card (NIC).
